Keep Them !
If possible, keep as much of the tile and as many fixtures as possible. Replacement parts are available for toilets. The tub and the sink can be resurfaced if they are worn and stained. The biggest problem can be the condition of the faucets, but they can usually be replaced. If not, the entire basin may need to be replaced.
Built-in tub or tub/shower fixtures in need of replacement, should be replaced. Some newly manufactured ones are of high quality and styled to fit an older bathroom. It is often possible to reach tub or tub/shower fixtures from the wall behind the tile wall so tile repair won't be required.
If tile repair is necessary for other reasons, that is the time to seriously consider how much life is left in the in-wall fixtures. Tile repair and replacement are expensive and time consuming. So make this decision to replace the fixtures before any tile work is done. Do not get rushed into installing fixtures that are not aesthetically or functionally acceptable. It is not true that you can go back later to put on nicer looking handles and escutcheons. Each manufacturer's handles and trim only fit its own valves.
